Please bring your insurance card and any pertinent information regarding your coverage to each appointment.
We appreciate and encourage payment of your deductible, co-payment and any other liability at the time of service. This will assist in minimizing the cost of operating our office and your cost of care.
We accept most major credit cards for your convenience.
If you do not have insurance, payment is expected at the time of service. Payment assistance is available for patients who qualify. Please contact our office.

We participate with many insurance plans; however, we recommend that you call your insurance company to verify that we participate with your plan. Any deductible or co-payment due is expected at the time of service. Any portion not covered by your insurance plan is expected to be paid after payment by the insurance company.

Please bring your insurance card with you to every visit to ensure the correct information can be filed. We also ask that you fill out an updated patient registration form annually.
Please notify us if you have any changes in your name, phone number, address or insurance as soon as it occurs.
We are happy to assist you with your referrals, but we do consider it the patient's responsibility to notify us with the necessary information in a timely manner. We must be given at least 48 hours advance notice of your appointment to complete any required insurance referral. Same day referrals will only be made in cases of emergency.
